It comes down to the staircase, not the town. A straight run — common in the newer construction out on the east-side hills — usually installs for somewhere in the $2,000 to $4,500 range. Curved rail, which is what a lot of the older homes near City Park and downtown need because their stairwells were built narrower and often turn partway up, runs higher, generally $4,500 to $10,000 or more depending on how many bends the rail has to make.

A house built close to landmarks like the Carnegie Library or the Bank of Italy building wasn't designed with a stairlift in mind — the stairs are tighter and the landings are smaller, so the rail almost always has to be custom-curved rather than a straight off-the-shelf track. That's the main reason two quotes in the same city can come out pretty different.
We see more of them than you'd expect. With foothill lots rising toward the Santa Lucia Range on the west side of town, a fair number of homes have exterior stairs up to a raised entry or deck in addition to interior stairs, and those need weatherproofed rail and hardware rated for outdoor use, which adds to the project cost beyond a standard interior install.
A straight-rail job is usually done in half a day. Curved rail takes longer because it's measured and bent to the exact staircase — that's more common in the older houses around City Park with tighter, turning stairwells — so plan on the installer needing extra time to template the stairs before the actual install day.
No. The rail bolts to the treads of the stairs themselves, not the wall, so original woodwork and trim in the older downtown homes near National Register buildings like the Bank of Italy building stay untouched. That's actually one reason stairlifts get chosen over structural remodels in historic-adjacent houses — nothing about the house has to change.
Sometimes, on exterior installs. A staircase built into a sloped lot toward the eastern hills or the Santa Lucia foothills can have an uneven base or settled concrete, and the installer needs to account for that when mounting the bottom of the rail so the ride stays level top to bottom.
Possibly, through the Medi-Cal Community Supports Environmental Accessibility Adaptations benefit — it's a CalAIM add-on that managed care plans can choose to offer, capped at $7,500 over a member's lifetime for home modifications. In San Luis Obispo County that plan is CenCal Health, so you'd call them directly to confirm whether they've elected to offer it before counting on it.
